Why 1064 nm carries this job
Wavelength decides depth. Our technical training material defines penetration depth as the distance over which light falls to 37% of its entry intensity, and that distance grows with wavelength — longer light travels further before absorption and scattering strip it out. 532 nm gets consumed close to the surface. Excellent for a freckle. Close to useless against dermal melanophores.
1064 nm reaches. That single fact is why the whole melasma literature sits on Q-switched Nd:YAG and not on something greener.
The other half of the argument is monochromaticity. The same archive makes the point that selective photothermolysis only becomes possible because a laser emits one narrow band, so the beam can be matched to one absorber rather than heating everything in its path. Anderson and Parrish laid that principle out in Science in 1983 (PMID 6836297), and every pigment protocol written since has been an argument about how to apply it.
Low fluence is a technique, not a discount
Melasma punishes energy. Push the dose up chasing a fast result and you get rebound darkening or post-inflammatory pigment instead of clearance — worse skin, angry patient, refund conversation. The published approach runs the other way entirely. It's called laser toning: 1064 nm Q-switched output at roughly 1-3 J/cm², a collimated large spot in the 6-10 mm range, 5-10 Hz, repeated weekly or biweekly for around ten sessions. Those figures come from a 2022 systematic review by Lee and colleagues in Medicina, which pooled studies published from 2009 onward and found real-world fluences scattered between 0.5 and 3.8 J/cm².
The arithmetic your operator has to hold in their head
Fluence is energy divided by spot area. Our archive writes it out as energy density = (power × time) / spot area, which sounds academic until you notice the consequence: halve the spot diameter and the same millijoule setting delivers roughly four times the dose to skin. That's how a toning pass becomes a burn without anybody changing the energy number on the panel.
Spot size does something else too. At the same wavelength and the same fluence, a bigger spot penetrates deeper, because proportionally less light scatters out through the beam edges. Our device manuals push spots above 2 mm for anything sitting in the dermis and treat sub-2 mm apertures as a narrow special case. For melasma specifically, the manual guidance is no less than 6 mm.
Treatment protocol
Test patch first. Every time.
Treat a small square, 0.5 to 1 cm², then wait several days. Our internal atlas is blunt about why: some melasma patients throw obvious pigmentation after a pass, and it only settles later. You cannot predict which patient you have from a consultation form. If the test area darkens and stays dark, stop lasering and manage topically — vitamin C, hydroquinone, tretinoin under a dermatologist's direction, whatever your market allows.
What a real course looks like
- Wavelength: 1064 nm. Don't reach for 532 nm unless you've confirmed the pigment is purely epidermal, and even then be careful in darker skin.
- Spot: as large as the handpiece will give you. Our operator manuals specify no less than 6 mm for melasma.
- Energy: start low and creep upward on tissue response, not on a printed chart. Manual protocol opens around 200 mJ at a large spot and adjusts from there.
- Rhythm: roughly one session a week, ten to fifteen sessions before you judge the outcome. Fewer than five passes per session in the reviewed literature.
- Endpoint: faint pink. Nothing more. Pinpoint bleeding or pigment blanching white means you overshot, and that's the direct route to lasting hyperpigmentation.
Skin must be dry when you fire. A wet surface bleeds energy away and your delivered dose stops matching the panel reading — small detail, big effect on session-to-session consistency.
Aftercare decides half the result
Sun undoes the work faster than any laser fixes it. The American Academy of Dermatology recommends a tinted sunscreen containing iron oxide at SPF 30 or above for melasma, because visible light — not only UV — drives the pigment, and untinted formulas don't block it. Add topical vitamin C, a moisture mask, and a genuine conversation about hats and shade. Our manuals also flag hormonal and lifestyle drivers; treating the face while ignoring the contraceptive or the daily commute at noon is a plan that fails slowly.
The complication you should quote to buyers
Mottled hypopigmentation is the signature adverse event of toning, and it can persist. In one retrospective series covered by that Medicina review, it appeared in 21 of 177 patients within ten sessions, with excessive cumulative energy identified as the associated factor. Rebound hyperpigmentation runs the other direction, and it hits darker skin hardest. QE-01 against QN-03, honestly
Two Q-switched Nd:YAG platforms sit on this site and they are not interchangeable for this indication. The QE-01 is electro-optic Q-switched: 6 ns pulse width, 1064 and 532 nm, spot continuously adjustable from 1 to 7 mm, 1-10 Hz. Two of those numbers matter for melasma. Continuous spot adjustment lets an operator sit at the large end of the range where toning lives, and the higher repetition rate matches the 5-10 Hz the literature uses. Our engineering archive also records a flat-top beam profile on this class, which is what you want when a hot centre would mean an uneven dose across the pass.
The QN-03 steps its spot through fixed sizes from 1 to 4 mm, runs 6-8 ns at 1-5 Hz, and delivers up to 400 mJ at 1064 nm with stacked pulse modes above that. It's a focused pigment and ink instrument first — tattoos, sunspots, discrete lesions — with melasma as a secondary capability. See freckle removal for the work it does best.
The QE-01's standard handpiece reaches 6-7 mm, inside the published 6-10 mm window; the QN-03 tops out at 4 mm and misses it. Collimation, a separate property from spot diameter, is unstated on the datasheets. So ask for the collimated large-spot handpiece option in writing, and get the spot diameters and beam profile listed on the quotation.
Prep, adjuncts, and one thing not to reach for
Deep cleansing before a pass appears in every version of our protocol. A hydro-oxygen system such as the OJ-01 handles that side of the room.
Carbon-assisted toning is a related but separate protocol — carbon lotion worked into the pores, then 1064 nm at low single-pulse energy, roughly 20-60 mJ at 3-5 Hz per our process notes. It targets pores, tone and texture. Don't let a distributor sell it to a melasma patient as the same treatment.
And resist the urge to solve melasma with pulsed light. Broadband systems heat epidermal melanin across a wide band, which is close to the opposite of what a melasma epidermis needs.
What to check before the crate ships
Ask for the safety file. The governing standard for this equipment class is IEC 60601-2-22, fourth edition 2019, which sets particular requirements for surgical, cosmetic and therapeutic laser equipment on top of the general IEC 60601-1 requirements — interlocks, key switch, emission indicators, footswitch behaviour, labelling. CE marking for the EU leans on it. Get the declaration of conformity and the test report at quotation stage, not after the crate lands in customs.
One practical item people forget: eyewear. A dual-wavelength unit needs protection covering roughly 200-1080 nm, because 532 nm is visible and 1064 nm is not, and the invisible one is the one that blinds quietly. Frequently asked questions
Can laser permanently cure melasma?
No. Anyone promising that is either untrained or selling. Melasma is chronic, and relapse is listed as a complication in our own clinical reference material. Low-fluence 1064 nm toning can lighten the patches meaningfully and hold them under control, but hormones and sunlight bring pigment back. Frame it to patients as ongoing management with maintenance sessions. That framing also happens to build a better repeat-revenue model than a false promise does.
Why not use higher energy and finish in three sessions?
Because melasma reacts to aggression by darkening. High fluence triggers rebound hyperpigmentation, and the endpoint you'd be chasing — pigment blanching or pinpoint bleeding — is exactly what our manuals warn leads to serious pigmentation afterwards. The slow route isn't timidity. It's the only route with a published safety record behind it. Ten to fifteen gentle sessions beat three heroic ones every time.
How do settings change for Fitzpatrick IV-VI skin?
More epidermal melanin means more competing absorption, so the same panel setting delivers a harsher experience to darker skin. Work at the low end of the fluence range, use the largest spot available, extend the intervals, and treat the test patch result as binding rather than advisory. Rebound hyperpigmentation is documented as more likely in darker skin. An experienced operator matters more here than any number on the sheet.
What separates a toning-capable machine from one that merely has 1064 nm?
Three things. Spot size at the large end, 6 mm and up, ideally collimated. A beam profile flat enough that the centre isn't running hotter than the edge. And a repetition rate high enough to lay down even coverage at 5-10 Hz without the operator crawling across the face. Pulse width in the 6-8 ns band is table stakes for any pigment work. Check all of it against the specification table, not the brochure headline.
Do I need separate machines for pigment removal and melasma?
Usually not, if you buy the right one. A Q-switched Nd:YAG platform with a wide spot range covers discrete pigment at small spots and toning at large spots on the same cavity. Where clinics get caught is buying a unit whose spot tops out at 3 or 4 mm, then discovering the toning protocol they were sold needs double that. Confirm the handpiece inventory before you sign, and ask what lamp and consumable replacement looks like at your projected shot volume.
